What Happens After You Apply
Step 1 — Application Review
I’ll review your application to ensure that this type of support is appropriate and within the scope of nurse-led education and navigation services.
Step 2 — Connection Call
If it appears to be a potential fit, you’ll be invited to a 30-minute call. This conversation allows us to talk through where you are in your care, what feels most overwhelming, and which program option aligns best with your current needs.
Step 3 — Next Steps
After our conversation, we’ll determine whether Thrive Through or Thrive After is the most appropriate option based on your treatment phase and goals.
If this support is not the right fit, I’ll share general suggestions for other non-medical resources that may be helpful.
