For New Diagnosis & Active Treatment
12–16 weeks
Nurse-led education and navigation support designed to reduce overwhelm, help you prepare for appointments, and provide steady support alongside active treatment.
This program is for individuals who are newly diagnosed, early in treatment, or navigating ongoing or changing treatment plans and want informed, non-clinical support throughout the process.

How we choose your path
1) Apply
You’ll complete a short application so I can learn more about your situation and ensure this type of support is appropriate and within my scope as a nurse.
2) Meet for a Session
If we’re a good fit, we’ll talk through what you’re facing and identify the program that aligns best with your current phase of care.
3) Begin with Clarity
You’ll receive education resources, navigation support, and tools tailored to where you are, whether that’s active treatment or the transition into survivorship.
