8 Weeks
Support for the transition from active treatment into survivorship.
Organize follow-up care, prepare for surveillance, navigate questions and uncertainty, and find your footing when treatment ends but the cancer experience doesn't simply disappear.

Northbound Roots provides nurse-led education, navigation, organization, appointment preparation, communication support, and general cancer-related education.

Northbound Roots does not diagnose conditions, interpret individual scans or laboratory results, prescribe or recommend treatment, assess symptoms, provide emergency care, or replace your oncology team.
